Athletics & Activities
To get your student cleared for athletic activities, simply go to the FinalForms website and complete the EPSD9 Athletic/Activity clearance process.
After you’ve finished the yearly district school registration, you’ll get an email from FinalForms to claim your account (just for new users) and set up a password. This email will automatically go to the email address you used when you registered. Once you’ve claimed your account, you’ll need to fill out all the digital forms, upload a current athletic physical (make sure you use the right OSAA form, if needed), and pick the sports and activities your student wants to join. This page will give you all the links, info, and help you need to complete the FinalForms clearance process.

Important: you’ll need to digitally sign all the forms in FinalForms to get your student cleared for athletic participation. (For the liability insurance form, you don’t need to fill out any details; just add your digital signature and move on to the next form.)
